| Abstract | 「はじめに」 急性アキレス腱断裂の手術は, 断裂したアキレス腱の断端を確認し縫合する手技である. アキレス腱縫合術は, 断裂した腱の端を露出させ縫合糸を使用して端を結ぶ, 一見単純な手術のように思われる. しかしアキレス腱縫合術は, 腱の緊張が強すぎたり弱すぎたりすると筋力や足関節可動域に影響を及ぼすため, アキレス腱を適切な緊張で縫合するように注意が必要である. またアキレス腱のみではなく, 皮膚やパラテノンなど軟部組織を慎重に取り扱わないと, 感染や皮膚の損傷などの深刻な問題が発生する可能性がある. アキレス腱外側にある腓腹神経の損傷は, 感覚の障害や疼痛を引き起こす. 本稿では, 新鮮アキレス腱断裂における手術療法の基本的な考え方と手技に関して詳述する. 「新鮮アキレス腱断裂における手術療法の適応」 新鮮アキレス腱断裂における手術適応は, 年齢や性別, 活動性を問わず手術療法を理解することができ, 早期スポーツ復帰や社会復帰を目指す人が対象となる. |
