| Abstract | 「はじめに」上腕骨近位部骨折に対しては, 骨折型に加えて骨質や生活レベル, 合併疾患などの患者因子も考慮して治療法を選択する. 保存的治療により疼痛残存や機能的損失が危惧される場合は手術的治療の適応となるが, 高齢者においても上腕骨頭は可能な限り温存すべきである. しかし, 骨頭壊死が懸念される症例に対しては, 人工骨頭置換術(humeral head replacement:HHR)の適応となるが, その成績はexcellentとpoorの両極に分かれる二峰性である. HHRの成績不良因子としては, 高齢者, 脱臼骨折, 腱板断裂が挙げられ, また骨質不良に伴う大小結節骨片粉砕例では骨癒合不全や骨吸収が起こり, 良好な機能獲得が困難となる. リバース型人工肩関節(reverse shoulder arthroplasty:RSA)は腱板断裂性肩関節症(cuff tear arthropathy:CTA)に対して開発された人工関節である. |
