| Abstract | 「はじめに」大腿骨頚部/転子部骨折については, 2021年3月に日本整形外科学会から診療ガイドライン2021(改訂第3版)が出版された. また, 英国麻酔科学会からマネジメントガイドライン2020が発表された. 本章では, 日本と英国の医療事情を踏まえつつ, 周術期の合併症予防の要点, 特にセメント使用の安全性と術中の注意事項についてレビューしたい. 「1. 全身麻酔と区域麻酔(脊椎・硬膜外麻酔)のどちらが良いか」日整会ガイドライン2021では,「全身麻酔と区域麻酔(脊椎・硬膜外麻酔)において合併症および死亡率に差はない」としている. 英国麻酔科学会ガイドライン2020では, 外傷, 手術, 老年病的なケアや患者要因(年齢, 虚弱, 認知障害)による影響に比べて, 麻酔方法の違いによる影響は小さいとしている. |
