Abstract | 「はじめに」近年の高齢者人口増加, 大腿骨頚部骨折, 変形性股関節置換術に対する人工骨頭置換術(BHP), 人工股関節置換術(THA)の増加に伴い, インプラント周囲骨折も増加の傾向にある. 本稿ではBHPだけでなくTHAのステム周囲骨折も含めて基本事項を解説する. 本邦ではBHPとTHA併せて年間約10万件以上施行されている. インプラント周囲骨折の頻度は0.1%から18%と報告されており, どの施設でも遭遇する可能性がある. 特に大腿骨頚部骨折後のステム周囲骨折においては, 患者の多くが基礎疾患を持ち, 骨粗鬆症が背景にあることを念頭に, 骨接合術だけでなく既存のインプラントの再置換も行わなければならない. そのため治療においては骨折治療と人工関節再置換術の正確な知識と技術が必要とされる. |
