| Abstract | 「半月板治療の変遷: 切除術から縫合術への変遷」最初の半月板手術は文献上, 1883年に Annandale が関節切開により内側半月板縫合術を行ったもので, 1885年にその手術手技と術後経過を症例報告している. 半月板切除の最初の報告も Annandale によるもので, 1889年に関節切開下に内側半月板切除術を行った1例が報告されている. その後, 1890年代には縫合術と切除術の両方が行われていたが, 半月板機能の重要性が認識されていなかったこともあり, 縫合術が定着することなく, 短期の成績が良好である全切除術が一般的になった. 1936年に King らは動物実験により半月板切除術は関節軟骨に変形性変化を引き起こし, その変性の程度は切除量に相関することを報告した. さらに1948年には Fairbank が半月板切除術後14年までのX線で, 内側半月板切除では32%, 外側半月板では40%に関節裂隙の狭小化が見られたことを報告した. |
