Abstract | 「はじめに」半月板逸脱の原因として, 半月板後根断裂や放射状断裂, 半月板切除後, 変形性膝関節症(osteoarthritis, 以下OA)による骨棘(軟骨棘)形成などが挙げられる. 半月板の逸脱は荷重分散に重要なhoop機能の低下を意味し, 関節軟骨に対する負荷が増大することにより, OAの進行やOA患者の膝痛と相関する. 鏡視下Centralization法はアンカーを用いて逸脱した半月板を内方化させる手術方法であり, 短期ではあるが良好な成績が得られている. また, バイオメカニクスの検証からも, Centralizationにより半月板の荷重分散機能を再獲得することが可能である. 本稿では, 外側半月板(LM)部分切除後に生じた逸脱に対して鏡視下Centralization法を施行した症例を提示し, 解説する. 「手術の概要・目的・適応」鏡視下Centralization法は, LM逸脱, 内側半月板(MM)逸脱の両方に適応がある. |
