| Abstract | 「はじめに」開放骨折は, 高エネルギー外傷であり, 単独外傷であっても, その出血がコントロールできなければ死に至ることがある. また, 血管損傷を合併することもあり, 血管損傷の診断, 治療が遅れれば, 阻血状態に至った四肢を切断せねばならない. 開放骨折初期治療において, 治療に従事する医師は, その治療優先順位を考え, 全身, 血管, 最後に骨折部という順に診察するべきである. 実際, 世界の外傷診療の基本となっているAdvanced Trauma Life Support(以下ATLS)において, 四肢外傷について, Life threatening injury, そしてLimb threatening injuryという言葉を使用し, それぞれ加療すべき疾患について解説をしている. 本邦においては, たとえ高エネルギー外傷に伴う開放骨折であっても, 四肢単独外傷であった場合には, 全身管理に不慣れな整形外科医が単独で初期治療を行うことも少なくない. |
