| Abstract | 「はじめに」開放骨折では, 損傷部の汚染, 損傷組織の血流不良, 進行する軟部の壊死, 病院環境への露出などが感染に進展するリスクを高めている. これを予防する戦略は, (1)抗菌薬, (2)デブリドマン, (3)洗浄, (4)骨再建, (5)軟部再建のすべての過程を正しく遅滞なく行うことである. 本稿では, 抗菌薬の全身投与の実際と, われわれが近年施行している高濃度抗菌薬局所投与について詳述する. 「いつ抗菌薬を投与するのか?」開放骨折において抗菌薬の先行投与が感染予防に有効であることは古くから報告されている. これは2004年にシステマティックレビューで科学的に証明され, Eastern Association for the Surgery of Trauma(EAST)のガイドラインにも引用された. この論文では, 四肢の開放骨折に対して初期治療時かその前に抗菌薬を投与された群645例が, プラセボ群461例に比べて感染率が0.43倍低いと報告している. |
