| Abstract | 「はじめに」足部は前足部, 中足部, 後足部に分けられる. 骨折外傷の頻度が比較的多いのは, 前足部では中足骨骨折, 前足部と中足部の境であるリスフラン部ではリスフラン脱臼骨折・関節損傷, 後足部では踵骨骨折, 距骨骨折である. 脛骨骨幹部および骨幹端は皮膚が薄く開放骨折となりやすい. 脛骨骨幹部骨折のうち, 24%が開放骨折とされている. 腓骨骨折を併発することにより不安定性が増し, 初療時に創外固定を施行することが多い部位である. 本稿では, それぞれにおける症例を提示し, 経皮的な手技の適応と有用性を中心に述べていく. 「足部および足関節周囲骨折の手術適応」それぞれの部位での手術適応は違うが, 大まかに述べると(1)転位が大きい骨折, (2)関節面との適合性が断たれている骨折, (3)アライメントの変形により後遺障害が出現する恐れがある骨折が手術適応となる. また, 足部や下腿遠位部の軟部組織は薄いため注意が必要である. |
