| Abstract | 「はじめに」表題は「重度肘関節外傷の治療戦略」であるが, その内実は「複合性肘関節不安定症(Complex Elbow Instability)」である. この損傷は, 尺骨鉤状突起骨折を中心として, 橈骨頭骨折や肘頭骨折などの骨損傷に加えて, 筋腱靱帯損傷を伴うものである. 同損傷の発生頻度は低く, その損傷病態の複雑さにより, 経験豊富な臨床医であっても治療方法の立案とその遂行は容易ではない. 治療の目指すところは, 安定した求心位可動を有する肘関節の再獲得であるが, そのためには骨折部の解剖学整復固定に加えて, 筋腱靱帯の修復を的確に行うことが必要である. 近年の機能解剖学的研究により, 肘関節の一次安定化機構(腕尺関節, 内側側副靱帯, 外側側副靱帯)と二次安定化機構(前方関節包, 橈骨頭, 鉤状突起)の理解が明瞭となり, どの組織を如何に修復すべきかの治療プロトコールが提唱されてきた. |
