Abstract | 「はじめに」肘関節の安定性には骨性要素と靱帯性要素が重要であるが, 尺骨鉤状突起骨折はその両方を含む骨折である. 両要素の破綻を伴うcomplex elbow instability(CEI)の病態において, 尺骨鉤状突起骨折はその根幹をなすものとされる. 50%を超えるような鉤状突起骨折に関しては固定すべきとしてコンセンサスが得られているが, それ以下の小〜中骨片を固定すべきかどうかは議論が分かれている. 本稿では最近の知見にわれわれの経験を踏まえて解説した. 「分類・受傷起点」Regan Morrey分類とO'Driscoll分類が広く使用されている. Regan Morrey分類は骨片の大きさによって分類されており, シンプルであるが簡にして要を得た分類である. O'Driscoll分類は大きさのみならず部位や受傷起点を考慮しており, より臨床に沿った分類である. 鉤状突起の骨折型と受傷メカニズムの間には高い関係性がある. |
