| Abstract | 「はじめに」橈骨頭骨折は日常比較的よく経験する骨折である. Terrible triad injury(TTI)を始めとする肘関節複合不安定症や肘頭脱臼骨折に合併することも多く, 骨折型, 年齢, 重症度によって治療が異なってくる. 合併損傷については他稿を参照していただき, 本稿では橈骨頭単独骨折の手術適応骨折型, アプローチ, 内固定材料, 人工橈骨頭置換術について解説する. 「骨折型」橈骨頭骨折において一般に頻用されているのがMason分類であり, 重症度により3typeに分け, さらにMorreyが肘関節脱臼を伴うものをtype 4とし追加した. Type 1は転位のない単純骨折, type 2は転位を有する関節内骨折, type 3は3つ以上の骨片に分かれた粉砕骨折である. 著者は橈骨頭骨折に頚部骨折を合併している場合, 骨膜の連続性が破綻した例を不安定型, 温存されている例を安定型とし, 大きく2つに分け治療方針を決めている. |
