Abstract | 「はじめに」肘関節外傷後の肘関節拘縮は非常に高率に起こる合併症の一つである. 日常生活に必要な肘関節の機能的可動域は伸展-30°・屈曲130°, 回内外それぞれ50°の全可動域100°とされている. 肘関節の機能的可動域喪失は肘関節機能のみでなく上肢全体の重度の機能障害となり得るため, その発生の予防および治療が重要となる. 本稿では肘関節外傷後の肘関節拘縮の病態とそれぞれの病態に対する治療法, 手術のタイミング, アプローチの選択, 術後後療法について述べる. 「肘関節拘縮の病態」肘関節拘縮の原因や部位などにより, いくつかの分類が提唱されている. 主なものはMorreyとKayによる分類である. Morreyらはその病因と解剖学的要素に基づいてintrinsicとextrinsicの2つのグループに分類している. Intrinsicな要因としては, 変形治癒した関節面, 関節内の遊離体, 骨棘, 関節軟骨欠損, 滑膜炎などが挙げられる. |
