| Abstract | 「はじめに」足関節の脛骨遠位端骨折(ピロン骨折)に対する基本的な手術治療方法は観血的整復および内固定(ORIF)である. しかし脛骨関節面が高度に粉砕していると関節症変化をきたし, 高エネルギー外傷などで軟部組織の状態が悪ければ皮膚壊死や感染などの合併症を引き起こす可能性が高くなる. また遷延癒合や関節症変化のために追加処置を行う際のリスクも高くなることが知られている. このため当院では症例によって一期的に足関節固定を行う. この際にどのような症例に適応するか, 固定した関節の癒合の成否や合併症, 固定方法の選択はどうするか, 問題点はさまざまである. 当院での実際の症例を提示しながら解説していきたい. 「手術の実際」「症例提示」50歳代, 男性. 自宅の2階より転落受傷. 同日前医にて右下腿開放骨折と診断され, 近医で初期治療後に加療目的で受傷2日目に当院紹介となった. |
