| Abstract | 「はじめに」こどもの大腿骨骨幹部骨折の治療法は, 他の骨折と比べて選択肢が多い. 年齢, 性別, 体重, 骨折の部位および型, 転位量および方向, 患者側の要望などにより決定する. 本稿では, 本誌の特性を鑑み, 治療選択肢の説明と, 多くの症例を供覧することを優先する. また, 最近適応が推奨されているElastic Stable Intramedullary Nail(ESIN)について詳しく解説する. 乳幼児の骨折に関して, 児童虐待に注意が必要であることは繰り返し述べられているところであり, 本稿では割愛する. 「治療戦略」大腿骨骨幹部骨折は高エネルギー外傷のことが多く, 全身状態と, 他の合併損傷がないかの評価を怠ってはならない. 低エネルギー外傷例や低年齢であれば, 児童虐待や病的骨折の鑑別が必要である. こどもの大腿骨骨幹部骨折の治療方法としては, 保存的治療および手術的治療として下記のように複数の選択肢がある. |
