| Abstract | 「はじめに」足関節および脛骨腓骨遠位骨端線の損傷は, すべての骨端線損傷の25〜38%とされ, 橈骨遠位の次に多い骨端線損傷である. 特にスポーツによる受傷が多く, 58%の骨端線損傷はスポーツ時に生じている. また, スポーツ外傷の10〜40%は, 骨が未成熟なアスリートに生じている. 「受傷のメカニズムと分類」足関節周囲の外傷には, 骨幹端部骨折, 骨端線損傷および骨端部骨折が含まれる. 骨端線および骨端部損傷はSalter-Harris分類(以下, S-H分類)I〜Vに分類される. 伸展力によって生じたtype Iに比べて, 剪断力や圧迫力によって生じたtype IIIやIVはより骨端線早期閉鎖の危険性が高い. 骨折の分類では, Lauge-Hansen分類をもとに作成されたDias-Tachdjian分類が受傷機転を基にした分類として使用される. Lauge-Hansen分類同様に受傷時の肢位と外力の方向によって示される. |
