| Abstract | 「本手術の基礎知識」 上腕骨顆上骨折は小児の肘関節周囲骨折のなかで最も頻度が高い骨折である. 肘関節を伸展した状態で手をつき, 過伸展を強制されて受傷する伸展骨折が大多数であり, 典型的な伸展骨折では遠位骨片が内反, 内旋, 伸展する. 肘関節を屈曲した状態で肘頭を強打して発生する屈曲骨折は稀である. 神経障害や循環障害を合併することが多いため, 術前後には必ず手指の知覚や運動, 循環状態を評価して記録する必要がある. 急患として若手医師が時間外に執刀する機会が多い骨折だと思われるが, 手術は決して簡単ではなく合併症の頻度も高いため, 十分な診療体制での手術を推奨する. 「適応」 Gartland分類でtype IIの転位が大きい例と, type IIIの例を徒手整復術および経皮的鋼線刺入術の適応としており, 原則として全身麻酔下に腹臥位での手術を行っている. 橈骨動脈の拍動を触知しないが手指の色調は保たれているpink pulseless hand例に対して, 上腕動脈を展開するか否かについては議論が続いている. |
