| Abstract | 「はじめに」足関節内果骨折は, 内果単独で骨折する場合や, 捻挫による足関節果部骨折の一部として起こる場合がある. いずれにしても変形が残存すると, 足関節の不安定性や疼痛, 変形性関節症への進行等が問題となる. そのため, 骨折の部位, 転位の量, 足関節の不安定性, 等を評価したうえで症例ごとに治療法, あるいは内固定方法を十分に検討し, 適切な治療を行っていかなければならない. 「内果骨折保存療法について」内果の単独骨折は足関節の骨折のうちわずか7%と少ないが, 外果や後果骨折を伴う骨折とは分けて考える必要がある. 外果や後果骨折を伴う骨折の場合は靱帯も損傷されていることが多く, ankle mortiseは破綻し不安定になっている. 一方, 内果単独骨折ではankle mortiseは破綻せず, 安定性が保たれていることが多い. また, 荷重という面でもそのほとんどは天蓋の中央部にかかるため, 内果単独の骨折では大きな影響はない. |
