Abstract | 「遠位脛腓靱帯結合の構造」遠位脛腓靱帯結合(シンデスモーシス)は, 脛骨の外側面である脛骨腓骨切痕のなかに腓骨がはまり込むような骨形状をしており, 靱帯性には遠位前方から前下脛腓靱帯(anterior inferior tibiofibular ligament:AITFL), 骨間靱帯(interosseous ligament:IOL), 後下脛腓靱帯(posterior inferior tibiofibular ligament:PITFL)が脛骨と腓骨をつなぐことで安定した構造になっている. 遠位脛腓関節は足関節背屈により腓骨が1〜2°外旋, 2mm開大する可動性がある. 「足関節果部骨折に伴うシンデスモーシス損傷」足関節果部骨折に併存するシンデスモーシス損傷は, AO分類44Bの17〜45%, 44Cの約80%に併存していると報告されている. シンデスモーシス損傷がある場合, 遠位脛腓関節の不安定性が生じる. |
