| Abstract | 「本手術の基礎知識」足部・足関節の疲労骨折では, 他の部位の疲労骨折と同様に保存療法が基本ではあるが, 競技レベルの高いアスリートなどでは手術治療が選択されることが多いhigh riskな特定の疲労骨折がいくつかある. そのうち比較的頻度が高く手術治療が行われることが多い第5中足骨基部疲労骨折(Jones骨折)を中心に, 足の舟状骨疲労骨折, 脛骨内果の疲労骨折の手術治療について解説する. Jones骨折は, 難治性であり手術しても偽関節・遷延治癒・再骨折が多くて治療・手術に難渋する. Jones骨折の多くは単純X線写真で診断されるが, 完全骨折の分類としてTorg分類がよく使われている. 手術治療は髄内スクリュー固定が一般的で有効である. 舟状骨疲労骨折は, 早期発見例では保存療法が有効であるが, 陳旧例・難治例・再発例ではスクリューやプレート固定と骨移植などの手術が必要になることもある. |
