| Abstract | 「はじめに」脛骨近位骨切り術におけるオープンウェッジ法は, 脛骨近位内側からヒンジ部に向かって骨切りした後に, 骨切り部内側を開大し, 内反アライメントを外反に矯正することで, 荷重ストレスを分散し, 痛みの改善と変形性関節症の進行を予防する術式である. 腓骨の骨切りは不要で, 脛骨の骨切りのみでよいために, 腓骨神経麻痺などのリスクがなく, シンプルで安全である. また, 術中に矯正角を自由に決定可能であるため, 正確な骨切りが可能であることが利点である. Staubliらが報告した"脛骨粗面を遠位骨片に残す"バイプレーン高位脛骨骨切り術(OWHTO)が世界中に広まり, 現在主流である. しかし, OWHTOでは術後膝蓋大腿(PF)関節の接触圧が増加し, PF関節OAが発症することが明らかとなり, "脛骨粗面を近位骨片に残し, PF関節の接触圧に影響を与えない"バイプレーン脛骨粗面下骨切り術(OWDTO)の適応も最近急速に広がっている. |
