| Abstract | 「通常の皮膚縫合の限界」切開された皮膚, 皮下組織は, 通常創縁は離開した状態になる, 決して, 何もしない状態で創縁が密着したような状態にはならない. つまり, 創部には創縁は開く向きの力がかかっている. これを通常の皮膚縫合を行っても縫合糸のかかった部位では, 創縁は密着するものの, 縫合の間では, 創縁は少しであるが開いた状態となる. これを少しでも解決するようにするには, 非常に細かいピッチで縫合するしかない. 通常の皮膚縫合では, 縫合糸に囲まれた組織は, それぞれが単独の皮弁と同じような血流動態となる. 狭いピッチで縫合される状態は, 細長い皮弁を作成したのと同じで, 皮弁遠位端の創縁での血流は悪くなる. したがって, このように縫合した場合, 血流障害で適切な創治癒が起こらず, 創がし開してしまうことはたびたび経験される. |
