| Abstract | 「はじめに」 鎖骨遠位端骨折に対するプレート固定術は良好な治療成績が報告される一方で, 遠位骨片が粉砕により十分な本数のスクリューを挿入困難な症例や, 下方に第3骨片を伴う症例は, 肩鎖関節をまたがないノンブリッジプレート単独では整復固定が困難となることがある. スコーピオンプレートはノンブリッジプレートの一種であるが, こうした通常のノンブリッジプレート単独では整復困難となる症例にも対応できるように本邦で開発されたプレートである. 本稿では, 手術適応となる鎖骨遠位端骨折のなかでも, 特に不安定と考えられる骨折型に対するSCORPION(R) NEO単独のプレート固定術の手術手技を紹介する. 「鎖骨遠位端骨折の手術適応と骨折型別の治療戦略」 鎖骨遠位端骨折の手術適応においては, 近位骨片と円錐靱帯の連続性の有無が重要となる. 骨片と円錐靱帯の連続性が途絶えたCraig分類Type II, Vの骨折型は保存療法で高率(22〜50%)に偽関節となることから, 一般的に手術適応と考えられている. |
