| Abstract | 「本手術の基礎知識」頚椎の変性に伴う脊柱管狭窄により発病する頚椎症性脊髄症に対して, 本邦では椎弓形成術が多く施行されている. 椎弓形成術には両開き式椎弓形成術, 片開き式椎弓形成術など多くの術式が含まれるが, いずれも脊柱管の減圧を目的とした手術法である. 本稿のメインテーマである椎管孔開窓術は, 脊柱管より外側の椎間孔部を除圧する手術術式である. 椎弓形成術に椎管孔開窓術を併施する目的は大きく分けて(1)治療目的, (2)予防目的に大別される. 「適応」われわれは, 椎弓形成術として片開き式頚椎椎弓形成術を行っているため, 本稿では, 同術式に椎管孔開窓術を併施する術式に関して説明する. 「(1)治療目的」最も典型的な頚椎症性脊髄症では両上下肢しびれや, 巧緻性障害をはじめとした運動障害が主症状である. しかし, 時に片側上肢の神経因性疼痛や筋力低下を伴う頚椎症性脊髄症を経験することがある. |
