| Abstract | 「はじめに」近年, 膝蓋骨不安定症患者における手術的加療の第一選択として内側膝蓋大腿靱帯(medial patellofemoral ligament:MPFL)再建術が行われている. MPFLは膝蓋骨制動の重要な構造物であり, 最近のシステマティックレビューから, 骨端線開存例におけるMPFL再建術の有効性が明らかとなっている. しかし, 移植腱, 術式など, まだまだ改善すべき点が多く存在する. 骨端線開存例におけるMPFL再建術の最大のポイントはMPFL大腿骨付着部の処置と骨端線損傷の回避にある. 大腿骨遠位における骨端線は大腿骨および下肢の成長に大きく関与することも報告されており, MPFL大腿骨付着部は骨端線近傍に存在するため, 骨端線温存に最大の注意を払う必要がある. 当科では大腿骨付着部に骨孔を作製し, 半腱様筋腱を用いた解剖学的MPFL再建術を報告している. |
