| Abstract | 「▼どんな手術?」 人工股関節全置換術(total hip arthroplasty:THA)とは, 股関節の骨盤側(寛骨臼)と大腿骨側(骨頭)を置換する手術です. 大腿骨だけの人工骨頭置換術(bipolar hip arthroplasty:BHA)とは異なり, 骨盤側も人工のカップに置換します. THAのほうがBHAよりも術後の疼痛が少なく, 可動性が良く, 再置換率が低いとされています. これにより, 変形性股関節症や関節リウマチなどの変性疾患だけでなく, 大腿骨頚部骨折に対しても早期に痛みをとる効果があり, 比較的長期(20年程度)にわたって安定した股関節機能が維持できます. [ひと言でいうと… 股関節の痛みをとり, より良く, 歩けるようになるための手術!] 「▼術前に注意すること」 術前の注意点はとくにありませんが, 大腿骨頚部骨折の場合, 骨折する直前まで歩けていた人(活動性が高い人)がTHAの適応になります(それ以外はBHA). |
