Abstract | 「現状と課題」非定型「大腿骨」骨折は, 骨密度増加や脆弱性骨折予防の効果がきわめて高いビスホスホネート製剤の長期使用などにより発症し, 骨癒合が遷延すると長期間の荷重制限を要する場合があるため, 臨床上きわめて重要な疾患である. 国内外で注目され活発に研究されているが, 多因子性疾患であるために複数の病態が混同して議論され, 未だ全容が解明されておらず, 明確な診断基準・治療指針がない. 筆者らは非定型大腿骨骨折の発症高位別特徴に注目し, サブタイプ分類 (Oh分類) を提唱した. 今後の研究でサブタイプ別に発症要因が同定され診断基準が明確になることで, 治療指針が確立し, 病態に即した治療法が選択できる可能性がある. |
