| Abstract | 「Point」●欧米と比べわが国では変形性肩関節症は少ないが, 2014年腱板断裂症性肩関節症に対するリバース型人工肩関節置換術(reverse shoulder arthroplasty;RSA)の導入により, 人工関節の症例が増えている. ●解剖学的人工肩関節とRSAの判断基準は年齢, 腱板断裂の有無, そして肩甲骨関節窩の変形によって決まる. ●RSAは良好な成績が得られるが, 腱板付着部を切断する術式であり, 肩関節治療の最終手段の位置付けであることを認識すべきである. ●変形性肘関節に対する人工肘関節置換術の適応は, 肩関節よりもさらに少ない. ●人工肘関節置換術では, 軟部組織バランス, 上腕三頭筋機能不全, 尺骨神経障害に注意すべきである. |
