| Abstract | 「Abstract」壊死性筋膜炎やガス壊疽を包括する疾患名として,最近では「壊死性軟部組織感染症」が使用されるようになっている. 症例によって進展速度が異なり, 劇症型 (時間の単位で進行), 急性型 (日), 亜急性型 (日〜週) に分類される. 特に, 劇症型では早期診断・早期治療が救命率に直結する. 症例によって, 臨床像にも多様性があるが, 壊死性軟部組織感染症に共通する治療方針として, 十分な抗菌薬投与, 迅速な外科的治療, 適切な全身管理が挙げられる, 本稿では, 治療の3本柱の1つである抗菌薬の投与法について, 起因菌判明前のempiric therapyと, 判明後のdefinitive therapyに分けて解説する. Definitive therapyでは, 単独で劇症型の壊死性軟部組織感染症を起こす代表菌であるβ溶血性レンサ球菌, ビブリオ・バルニフィカス, エロモナス属についてまとめた. |
