| Abstract | 「Abstract」皮膚軟部組織感染症に対する抗菌薬の選択について, 海外のガイドラインをもとに本邦の現状を踏まえて解説する. まず, 膿がみられ局所培養で起因菌同定がしやすい, せつ・よう・皮下膿瘍(purulent infection)と局所培養が困難な丹毒・蜂窩織炎(nonpurulent infection)に分類し, 適切な抗菌薬を選択する. 壊死性軟部組織感染症では, 抗菌薬投与はあくまで治療の一部であり, 迅速な外科的介入, 適切な全身管理が同時に必要とされる. また, 近年増加の一方である糖尿病足病変については, 別項目で検討する. |
