| Abstract | 「Abstract」水疱性類天疱瘡(BP)は最も頻度の高い自己免疫性水疱症で, 緊満性水疱と掻痒を伴う浮腫性紅斑を臨床的特徴とする. 重症度は症例によってまちまちで, ときにステロイド抵抗性の難治例に遭遇する. 難治例の治療選択肢はステロイドパルス療法や血漿交換療法, IVIG療法, 免疫抑制剤などに限られるが, 海外ではオマリズマブやデュピルマブ, リツキシマブの奏効例が報告されている. また, 本症ではストロンゲストクラスのステロイド外用治療が奏効するので, 重症例や難治例においても積極的に用いるべきである. 本症は高齢者に好発するため内服ステロイドの総投与量を極力減らすことが望ましく, 今後, 既存の生物学的製剤の適用拡大や疾患特異的新規治療薬の開発が望まれる. |
