| Abstract | 「Abstract」先天性サイトメガロウイルス(CMV)感染症の臨床像のうち, 難聴はもっとも重要なものの1つである. 新生児聴覚スクリーニングでreferだった場合には先天性CMV感染症を疑い, 必ず生後21日以内に尿CMV核酸定性検査を提出する. 診断が確定したら, 血液検査や頭部MRI, ABRなどで評価を行ったうえで, 中枢神経障害や顕著な臓器障害がある場合にはガンシクロビルまたはバルガンシクロビルによる抗ウイルス療法を生後1ヶ月以内に開始し, 6ヶ月間行うことが推奨されている. 副作用として好中球減少や肝機能異常が問題となり, 許容できない場合は休薬を要する. 一方で, これらの副作用は先天性CMV感染症の症状として出現することもあるため, 治療の適正化のためには全血中のウイルス量測定(real-time PCR)と薬物血中濃度測定が望ましいが, どちらも保険適用外である. |
