| Abstract | 「Abstract」慢性甲状腺炎(橋本病)は甲状腺濾胞上皮細胞を標的とする臓器特異的自己免疫疾患である. 様々な程度のびまん性甲状腺腫大と甲状腺自己抗体(抗サイログロブリン抗体, 抗甲状腺ペルオキシダーゼ抗体)の証明が診断に必要である. 主な治療対象は原発性甲状腺機能低下症だが, 甲状腺機能正常もしくは自覚症状を伴わず診断されることも少なくない. 甲状腺機能低下症はヨウ素過剰例では, ヨウ素制限で甲状腺機能が回復することもある. 甲状腺ホルモン補充療法の適応は年齢や妊娠の有無などを考慮して決定する. 治療薬はレボチロキシンNa内服を中心に少量より開始し, 2〜4週間毎に漸増するのが通常である. 一過性に甲状腺部の自発痛や圧痛を伴わず破壊性甲状腺中毒症を呈する無痛性甲状腺炎, 甲状腺萎縮が先行し阻害型TSH受容体抗体が証明される萎縮性甲状腺炎, 救急医療で遭遇しうる粘液水腫性昏睡など関連する病態にも留意すべきである. |
