| Abstract | 「Abstract」 アキレス腱断裂の治療の目的は, 腱長を保ちながら, 断裂したアキレス腱を早期に組織修復に導き, 下腿三頭筋の機能を再獲得することである. 治療時の重大な合併症に再断裂と腱延長による筋力低下がある. 近年, 早期運動療法の導入により, 再断裂率は低下しているが, さらなる成績の向上のためには, 治療中の腱延長を回避することが重要である. そのため腱の修復過程において安静にする時期と積極的にリハビリテーション治療を行う時期の違いを理解しておく必要がある. 手術は腱断端を縫合するため, 腱断端の離開に対しては有利であると思われるが, 感染などの重大な合併症もあるので, 低侵襲な手術により, 感染率を低下させる必要がある. アキレス腱断裂の治療にかかわる医療人は, 治療法の要であるリハビリテーション治療を理解することが, 保存療法, 手術療法を成功に導くために非常に重要である. |
