| Abstract | 「Abstract」 加齢, 外傷, 過体重, 神経障害などにより, 足趾・前足部に変形が生じ, 立位・歩行障害を生じる. 日常診療では足趾・前足部変形に伴う障害として強剛母趾とモートン病によく遭遇する. 強剛母趾は母趾MTP関節の変形性関節症である. 母趾MTP関節において第1中足骨頭背側の骨棘による可動域制限が特徴的である. 装具療法は靴の選択が重要であり, ロッカーボトム, トゥボックスの高い靴を履くように指導する. 足底挿板も効果的である. モートン病は総足底趾神経の趾間部における絞扼性神経障害である. 外反母趾, 扁平足によく合併する障害である. 趾間のみの感覚鈍麻が特徴的な他覚的感覚障害であり, 診断に有用である. 運動療法では, 後脛骨筋や長腓骨筋の筋力訓練は重要である. 装具療法では, 足がしっかりとホールドされる構造を有する靴を指導する. 足底挿板が最も効果的である. |
