| Abstract | 「Abstract」 呼吸リハビリテーションの中核である運動療法においても呼吸困難が制限因子となることが多く, この呼吸困難をいかに軽減させるかは呼吸リハビリテーションにおいて重要な課題の1つである. 呼吸困難のメカニズムは様々報告されているが, 中枢-末梢ミスマッチ説は呼吸筋ストレッチ体操に応用されており, motor command theoryは現在最も支持されているメカニズムの1つである. 呼吸困難に対する呼吸リハビリテーションとして, 呼吸練習(口すぼめ呼吸, 腹式呼吸), リラクセーション肢位, 呼吸介助, 呼吸筋トレーニング, 呼吸筋ストレッチ体操, 顔面の扇風機, 神経筋電気刺激などがある. Motor command theoryの呼吸運動出力を用いた検討においてリラクセーション肢位としてはセミファーラー肢位が推奨され, またリラクセーション肢位での呼吸介助併用効果も認められている. さらに呼吸筋トレーニングの効果として, 呼吸困難が軽減し, 最大吸気圧, 運動耐容能, 横隔膜筋厚も増加することが認められている. |
