| Abstract | [Abstract] 脳卒中による障害には, 脳そのものの損傷によって生じている一次障害(身体障害および高次脳機能障害)と, それより派生した患者と社会との関係によって生じる二次的障害(心理社会的障害)が生じる. いずれもが, 社会復帰を阻害する大きな要因である. 注意障害, 遂行機能障害, 記憶障害, 社会的行動障害(うつ, 意欲の低下, 病識低下, 易怒性等)などの高次脳機能障害に対し, 医療職は, 正確な診断, リハビリテーション治療(環境調整, 要素的訓練, 代償訓練, 行動変容療法, 認知行動療法, 社会技能訓練)および医療・地域連携, 自動車運転能力評価, 職業リハビリテーションなどを, 患者に沿って, 組み合わせて指導する. 支援者は, 患者の呈する社会的行動障害には意味があると捉え, 患者に寄り添いながら, 納得のもとで同じ方向に向き合う姿勢が大切である. |
