| Abstract | 「Abstract」心疾患, 中でも心不全患者は増加している. 心不全患者のリハビリテーションにおける早期離床の有効性が近年の多くの研究で示されている. 離床の安全性を評価するツールである急性期離床プログラムなどを用いて, 急性心不全もしくは慢性心不全の急性増悪患者の早期離床はリスク管理を行いながら段階的に進め, 運動療法まで移行する. 心臓リハビリテーションプログラムは運動療法のみでなく, 病態治療, 食事療法, 患者教育, カウンセリングが含まれ, 多面的・包括的な内容で構成される. 回復期から維持期に行う外来心臓リハビリテーションは運動耐容能や再入院率を改善する. しかし, 心不全患者の高齢化と, それに伴う社会的要因などから外来心臓リハビリテーションの継続が制限されることがある. 高齢化は心不全の増悪と深く関連しており, 心臓リハビリテーションの需要はさらに高まるだろう. 多様な疾患を併存する高齢心不全患者では個人に合わせた多職種での包括的かつ長期的な支援の工夫が課題である. |
