| Abstract | 「Abstract」心不全患者に対する運動療法を主体とした心臓リハビリテーションは, ランダム化比較対照試験のメタ解析によって, 運動耐容能やQOLの改善, 心不全による再入院リスクの低下に有効であることが明らかとなっている. しかし, 過去の心臓リハビリテーションの介入研究で予後をアウトカムにしたものは95%以上が心臓の収縮能が低下した心不全を対象とした比較的若い患者を主体としており, 現在, 日常臨床で問題となっている高齢フレイルの患者や左室駆出率が保たれた心不全患者(HFpEF)は対象に含まれていなかった. 本邦から最近報告された心臓リハビリテーションの実施と予後について調査した多施設コホートの研究結果では, 従来のエビデンスが確立されている患者に加え, HFpEFや軽度から中等度のフレイルを合併した心不全患者においても心臓リハビリテーションの実施が良好な予後に関連していた. 心臓リハビリテーションの普及率は決して高くないため, 心臓リハビリテーションを心不全の標準治療の1つとしてさらに普及していくことが必要である. |
