| Abstract | 「Abstract」投球障害肩とは, 投球時の肩痛により, そのパフォーマンスが障害される病変の総称である. 発症原因の多様性と複数の器質的損傷部位が存在することから症候群とも言え, その診断と治療には困難を要し, 病態発生に関しても未だ一定のコンセンサスが得られていない. 筆者が考察している投球障害肩の病態発生メカニズムとして, 肩甲帯に注視すると, 投球動作の繰り返しにより, 肩関節後方タイトネスと前方不安定性が出現し, 加えて腱板機能不全と肩甲骨の上腕骨頭追従機能不全が生じる. これらがジェネレーターとなり, 投球動作でのクリティカル・ポイントで, 上腕骨頭の求心性低下(非生理的偏位)が引き起こされ, 結果として断裂や剥離といった解剖学的破綻, すなわち損傷へ至るものと推察する. なかでも肩関節後方タイトネスは, late cocking肢位で上腕骨頭を後上方へ偏位させ, internal impingementの増大や, 上方関節唇への強いpeel-backを引き起こし, 腱板や関節唇損傷発生の元凶となると考える. |
