| Abstract | [Abstract] 変形性膝関節症は中高年に多い加齢に伴う関節疾患で, 回復期リハビリテーション病棟の患者に高頻度に認められる. 症状として, 膝関節の痛み, 腫脹, ROM制限, 圧痛, 変形, 歩行障害を認める. 膝関節内側が障害されるタイプが多く, 膝の内反を呈し, 歩行時にlateral thrustを認めることがある. 診断・評価はX線撮影を用いて行う, 脳卒中患者に合併することが多く, 脳卒中発症後症状が増悪する患者も存在する. 痛みに対して, NSAIDsなどの鎮痛薬や, 湿布, 軟膏が使用される. ヒアルロン酸の関節内注射もしばしば施行される. 運動療法としては, 大腿四頭筋の筋力訓練が行われる. 装具療法としては, 支柱つきの軟性膝装具や外側ウェッジの足底装具を処方する. 痛みが強く転倒のリスクが高い患者では, 歩行器やシルバーカーの使用を検討する. |
