| Abstract | [Abstract] 橈骨遠位端骨折に対する保存療法では, 外固定期間中の浮腫への対応, MPやIP関節可動域の維持, 伸筋腱と屈筋腱の癒着防止目的に可及的早期からリハビリテーションを開始する. また, 外固定中の肩関節や肘関節の拘縮や周囲の筋萎縮予防のため, 治療早期から上肢全体のリハビリテーションについても注意を払う. 手術療法では掌側ロッキングプレート(VLP)を用いた強固な内固定術が有効な手術治療法として広く行われている. 術後リハビリテーションは早期運動療法が推奨されている. 一方, VLPは強い内固定力を有するが, 骨癒合過程は保存療法と同様であり, 負荷運動は骨癒合状態をみながら慎重に開始する. 三角線維軟骨複合体や手根骨間靱帯など合併損傷についてリハビリテーション開始前の評価が重要である. 上肢脆弱性骨折後患者では, 2次骨折予防を念頭に置いた薬物治療や全身のリハビリテーションについても留意する必要がある. |
