| Abstract | 「Abstract」歩行はヒトの運動の最も基本的なもので, 日常生活に欠かせないものである. ヒトの二足歩行は, 四足歩行と比べ, 重心が高いため不安定であるが, 動き出しやすく重心の上下動や左右への移動が少なく, エネルギー消費が少ない. 一方, 四肢や体幹に麻痺や拘縮, 痛みなどがあると, 通常とは異なった筋肉や関節の使い方をするので, 歩行は代償動作を伴い, 跛行をきたすことになる. リハビリテーション治療では, 代償動作なしに, 正常歩行に近づけるよう治療するが, 代償動作をしないと歩行できない場合も多く見られる. 脳卒中片麻痺による共同運動や痙縮が残存している場合の歩行や, 小児麻痺や, 筋炎, 腰部脊柱管狭窄症などによる下肢筋肉の麻痺による歩行を代償動作の観点から述べる. |
