| Abstract | 「Abstract」中高年者では肩関節痛を訴える人は多い. 原因疾患として, 肩関節周囲炎, 偽痛風, 上腕二頭筋長頭腱炎, 石灰沈着性腱板炎, 腱板損傷, 頚椎症性神経根症などがある. 問診, 診察(視診, 触診, 関節可動域測定), X線撮影, MRI検査, 超音波検査などを行って診断する. 上腕二頭筋長頭腱炎, 腱板損傷・インピンジメント症候群では, 特有なテストが存在する. 脳卒中患者の半数以上に肩関節痛が認められる. 麻痺が重度な患者では痛みが生じやすく, 残存しやすい. 肩関節の障害には, 肩関節亜脱臼, 肩手症候群, 腱板損傷, 肩関節拘縮などがある. 診断後, 薬物治療・注射療法や患者指導を行う. 肩関節のリハビリテーション治療として, ストレッチ・関節可動域訓練, 腱板機能訓練を施行する. セルフエクササイズも有用である. |
