| Abstract | 「Abstract」半側空間無視とは, 大脳半球病巣と対側の刺激に対して, 発見して報告したり, 反応したり, その方向を向いたりすることが障害される病態であり, 急性期を除けば右半球損傷後に生じる「左」半側空間無視が大半である. 無視症状は, 頭部や視線の動きを許した状況下で起こるため, あらゆる日常生活場面で問題が生じ得る. 代表的責任病巣は側頭 - 頭頂接合部付近であるが, 前頭葉, 側頭葉, 視床など様々な部位の損傷で無視が起こる. したがって, 右大脳半球の脳血管障害患者では, 病巣によらず, 半側空間無視を伴う可能性を考えて評価することが大切である. 半側空間無視の発現メカニズムの基本は, 空間性注意の一側への偏向にあると考えるのが一般的である. しかし, 空間性注意は, 感覚 - 運動協調を基盤として様々な脳機能の影響を受けて機能する. また, 半側空間無視の最終表現は, 方向性を持たない複数の認知機能の影響も受ける. これらのことを念頭に置いて, 無視症状を分析的にみるように心がけてほしい. |
