| Abstract | 「Abstract」スポーツ活動に伴う膝伸展機構損傷の多くは膝蓋骨骨折であり, 大腿四頭筋腱・膝蓋腱断裂は比較的稀であるが, 陳旧例では治療成績が劣るため, 早期診断および早期の手術治療が必要となる. 腱実質部にかけた非吸収糸を膝蓋骨骨孔にpull outする方法が一般的であるが, 近年は修復強度の向上を目的に, 自家腱や人工靱帯を用いた補強術も併用される傾向にある. リハビリテーションにおいては, 修復部の治癒を図りながら, 膝関節機能の回復を目指していくが, 近年では膝関節拘縮と大腿四頭筋力の早期回復を目的に, 術後伸展位固定期間を短縮し, 早期より可動域訓練を行っていく試みが成されている. スポーツ復帰を目指したプロトコルは, 術後の時期と各フェーズで達成すべき基準が設けられた5段階のプロセスより成る. 平均7か月で元の競技レベルでの復帰が可能とされているが, 膝前面痛や膝蓋大腿関節の轢音, X線での膝蓋骨骨棘形成が見られる症例も多く, 復帰後もパフォーマンスが持続できるように継続的なサポートを行っていく必要性がある. |
