Title 膝蓋骨脱臼に対するスポーツ復帰を目指したリハビリテーション診療

Abstract 「Abstract」膝蓋骨不安定症の病態は多岐にわたるため, 個々の病態に応じた治療法を検討する必要がある. 本邦では反復性膝蓋骨脱臼に対する手術治療としては内側膝蓋大腿靱帯(MPFL)再建術が第1選択となることが多いが, 脛骨粗面外方化のある症例では脛骨粗面骨切り術が選択される場合もある. 一方で若年者の初回膝蓋骨脱臼に対する第1選択はリハビリテーションを中心とした保存治療が原則である. 保存治療および術後リハビリテーションにおいても中心となるのは大腿四頭筋および股関節外転筋の筋力訓練となる. さらにアスリートではスポーツ復帰を目指してアジリティー訓練や動作指導, 競技特異的訓練へと包括的にリハビリテーションを進めていく必要がある. 本稿では膝蓋骨脱臼に対し推奨されるリハビリテーションの実際について紹介する.
