| Abstract | 「Abstract」新鮮屈筋腱・伸筋腱損傷に対するリハビリテーションについて述べる. Zone I〜IV屈筋腱損傷に対するリハビリテーションは, 早期自動運動療法 (EAM) の治療成績が最も良好であるが, その施行にあたっては強固な腱縫合法の施行と精通した後療法の実施が必要である. 一方, Zone Vについては, 合併する神経血管損傷などの理由から固定法が選択される場合が多い. 新鮮伸筋腱損傷に対するリハビリテーションは, 指背腱膜部のZone I〜Vと固有腱部のZone VI〜VIIで大別される. Zone I〜Vについては, 皮下断裂が多いこと, 縫合を行っても腱が薄く強固な縫合は困難であり固定法が選択される. Zone VI〜VIIについては, 腱は太く抗張力を持つ縫合が可能であるため, 固定法だけでなくEAMなどの選択肢もある. 屈筋腱・伸筋腱損傷のリハビリテーションにおいては, 腱縫合法と運動療法についてのバイオメカニクスを十分に理解したうえで治療を行う必要がある. |
