| Abstract | 「Abstract」寛骨臼骨折は整形外科医にとって馴染みの薄いアプローチの1つである. 正しいアプローチを選択するには, まず正確な診断が重要である. 前方アプローチには代表的なものがいくつかあるが, ilio-inguinal approachが最も重要かつ基本となるものである. 近年ではquadrilateral surfaceの整復, 固定の操作がしやすいmodified Stoppa approachやPararectus approachも用いられるようになってきた. 後方アプローチはKocher-Langenbech approachが基本となるが, 通常の股関節後方アプローチと似て非なるものであることを確認する必要がある. いずれのアプローチもポイントを抑えて順を踏んで進めていくことが安全だが, 万一多量出血をきたした際の準備を怠らないようにすべきである. |
