| Abstract | [Abstract] 不安定型の大腿骨転子部骨折に該当する大腿骨転子間骨折(AO分類31A3)の治療戦略はいまだ確立されておらず, インプラントの選択も曖昧なままである. 本稿では新分類をもとに逆斜骨折の定義を明らかにし, 大腿骨転子間骨折の具体的治療戦略を提唱した. Type I(lateral wall pattern)は通常の転子部骨折(AO分類A1.3〜A2.3)の対応でよく外側壁を支持するインプラントを選択する. Type II(transverse pattern)は整復位獲得と保持が難しいが横骨折であり太い径を選択するか横止めスクリューを2本挿入するなどの工夫をすればショートネイルで対応できる. Type IIIは真の逆斜骨折でありワイヤリングにより外側壁と骨幹部を一体化させロングネイルを選択する. あくまで私見だが, 読者の日常診療の一助になれば幸いである. |
